Acute pancreatitis is a rapid onset inflammatory process of the pancreas which may have local and systemic manifestations. The most common causes acute are gallstones alcohol use. Other include drug-induced. Sodium-glucose cotransporter-2 (SGLT2) inhibitors class oral hypoglycemic agents approved by FDA for treating diabetes heart failure. 56-year-old, female patient known to uncontrolled type 2 diabetes. Presented with complaint epigastric pain radiating back then become generalized abdominal pain, moderate in severity, not relieved analgesic, associated nausea vomiting. denied any history alcohol, smoking A review her medication list included sitagliptin 100 mg, metformin 2g, gliclazide 60 mg more recently, empagliflozin 10 was started seven weeks prior symptoms onset. Physical examination significant tenderness area. Laboratory workup revealed an elevated serum lipase 3851 units/L, amylase 3755 units\L CT scan shows mild fatty atrophy peri-pancreatic fat stranding free fluid adjacent tale body suggesting evidence pancreatitis. Abdominal ultrasound gall bladder without stones. admitted diagnosis underwent conservative management.
